Ready to order your test? Follow these six steps:

3. Contact Repeat Diagnostics Laboratory
4. Collect and ship blood specimens
5. Receive Verification of your blood specimen
6. Receive results

2. Complete the REQUISITION FORM
- Have the appropriate medical staff complete and sign the Telomere Length Measurement REQUISITION FORM (above)
- All sections must be filled in
3. Contact Repeat Diagnostics Laboratory
- Before collecting blood, contact Repeat Diagnostics by email at test@repeatdiagnostics.com to coordinate an appropriate shipping date
- It is important that blood specimens arrive in the lab within 48 hrs after collection
4. Collect and ship blood specimens
- In order to process the blood specimen for Telomere Length Measurements, Repeat Diagnostics requires the following information:
- Patient ID #
- Age
- Sex
- Date and time of collection
- Detailed information can be acquired by downloading the SPECIMEN COLLECTION AND SHIPPING PROCEDURE CHECKLIST (above) and the FedEx COMMERCIAL INVOICE FORM (above)
- Shipping costs are the responsibility of the sender. Repeat Diagnostics does not accept responsibility for shipping costs associated with samples sent to us
- NOTE: only send samples on MONDAYS, TUESDAYS OR WEDNESDAYS or they may not be received in time for proper processing
5. Receive Verification of your blood specimen
- Repeat Diagnostics will acknowledge receipt of blood specimens to you by email
6. Receive results
- Test results will be faxed and/or emailed to you within 4 to 5 weeks on average. Expedite service is available upon request for a fee. Please contact the lab at test@repeatdiagnostics.com for more information
- An invoice will be mailed to the hospital.
